What is usually going on

Gradual sensory loss is one of the better situations in older animal care, because animals adapt to it far better than people do. A dog that has lost most of its sight over a year usually navigates a familiar house confidently, and owners are often startled to find out how much vision has gone. Smell and spatial memory carry most of the load, and neither is affected.

That fact drives the entire practical approach: keep the map stable. Do not rearrange furniture. Keep bowls, beds and litter boxes exactly where they are. Block stairs, pools and drops. Leave doors either fully open or fully shut rather than half open. For a deaf animal, use hand signals and get their attention with a stamp on the floor or a light flicked on and off, and always wake them by touching a shoulder rather than a head. For a blind animal, talk before you touch, and use textured mats at thresholds so they can feel where they are.

The one caveat is that gradual loss should still be examined once. Cloudiness may be a cataract, which is sometimes operable, or lens change that needs nothing. Blindness with increased thirst can be diabetes. High blood pressure causes retinal detachment and is treatable if caught. Deafness is usually just deafness, but it is worth having the canals checked, because a chronic ear infection or a wax plug can be a reversible part of it. One appointment is a reasonable investment, and then the job is environmental.
